Job Overview
The autonomous nature of the role lets clinicians function like a private practice without the hassle of managing a back office.
Ability to work within a 40‑hour week; majority of time clinically focused, remainder on patient documentation and facility correspondence.
On‑site clinical provider working with residents and their families.
Active lead member on an interdisciplinary team addressing residents’ emotional, social, and psychological needs.
In charge of caseload, schedule, and ultimately salary.
Provide initial assessments, use psychotherapy to address mental health issues, and offer ongoing support.
Responsible for all documentation requirements and keeping up‑to‑date on Medicare and commercial insurance plans.
Key Requirements
Entrepreneurial spirit with strong medical knowledge, documentation (CMS, Medicare, Medicaid) and nursing home/hospital facility experience.
Master’s degree and valid LISW/LMFT/LMHC license in the state of application.
Two years of counseling and therapy experience.
Team player with a positive attitude.
Knowledge of mental health diagnoses and therapeutic approaches.
Experience collaborating with interdisciplinary teams, including MPAC providers, to ensure optimal patient care.
Fosters open communication with the facility team and Regional Clinical Manager to ensure success of clinical programs.
Initiative and effective communication across multiple settings and parties (patient, family, nursing staff, facility social services, MPAC providers, etc.).
High level of organization to manage patient needs, document effectively, and ensure mental health needs of residents are met.
Comfortable with the changing environment of a skilled nursing facility.
Additional Requirements
Knowledge of Medicare benefit and commercial insurance plans.
Experience in the healthcare setting.
Passion for working with older adults.
Exceptional organizational skills.
Experience in the nursing home space is a plus.
