Day City Hospice was founded by experienced corporate professionals who created a patient-centered hospice that maximizes resources at bedside while being operated locally. Our passion is to provide patients and their families with care rooted in the communities we serve. In Ohio, our local management team and clinicians respond to patient needs in a timely and appropriate manner so that every patient receives personalized attention. Why Join Us
We don’t require high caseloads or unmanageable quotas, giving our team more time with each patient. Our top priority is our patients and the people serving them. Schedule
3 12-hour shifts per week, including every other weekend. What We Offer

Provide direct patient care as defined in the State Nurse Practice Act Implement current nursing practice following a comprehensive assessment and the plan of care Provide accurate and timely documentation of patient services to reflect the plan of care Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self‑plan of care Participate in the coordination of hospice services, appropriately reporting identified needs to the interdisciplinary group Provide appropriate pain/symptom management and evaluate the patient’s response to treatments/medications Qualifications
Graduate of an accredited practical nurse or vocational nursing program Three years of nursing experience; community health, hospice, or medical/surgical experience preferred Currently licensed as an LPN/LVN in the practicing state Must possess current CPR certification Demonstrates good verbal and written communication and organization skills Valid driver’s license with a vehicle insured in accordance with state requirements Understands the philosophy of hospice care and the needs of the terminally ill Text 9863 to (937)863‑9487 to apply Apply
