Pay & Benefits Base pay range: $17.79 - $19.23 per hour. Position Summary Community Health Action of Staten Island (CHASI), in partnership with Sun River Health, runs the Ryan White Support Services program to improve HIV health outcomes. The Peer Advocate will mentor and guide people living with HIV, leveraging personal experience to enhance engagement and well‑being, and to help connect clients to needed services.
Duties & Responsibilities
Provide program overview, department and agency services through orientation activities;
Accompany participants to provider sites for support and navigation assistance;
Co‑facilitate group activities teaching clients to identify high‑risk thoughts and slow impulsive behaviors;
Assist with community outreach and program promotion as needed;
Provide food and nutrition counseling/education and complete required training;
Conduct telephone and office‑based follow‑ups to address barriers to care and treatment;
Give reminder phone calls to reinforce retention;
Follow up on referrals and engage participants in programming;
Outreach to participants who miss appointments;
Attend staff development trainings;
Participate in supervision and team meetings;
Complete all program documentation promptly;
Perform all functions in alignment with CHASI’s mission, vision, and core values;
Other duties as assigned.
Requirements
Holds a NYS AIDS Institute Certified Peer credential (preferred) or other peer certification, or ability to obtain certification within 6 months of hire;
Lived experience with HIV is highly preferred; experience with substance use and/or mental health conditions is welcomed;
High school diploma or GED preferred;
Bilingual in Spanish highly desired;
Understanding of community‑level work and collaboration with organizations;
Effective oral and written communication skills;
Ability to juggle priorities and manage time in a fast‑paced environment;
Knowledge of community resources in Staten Island;
Organizational and problem‑solving skills;
Proficiency in Microsoft Word, PowerPoint, and Excel;
Familiarity with electronic health records or willingness to learn.
