Qualifications Proficiency in Social Work and Medical Social Work with the ability to assess and address patient needs Experience in Case Management, including planning and coordinating services for patients Strong knowledge of Mental Health and Behavioral Health concepts, diagnosis, and interventions Excellent communication, active listening, and problem-solving skills Ability to work independently while collaborating with healthcare teams remotely Relevant licensure as a Licensed Clinical Social Worker (LCSW) in the applicable state Familiarity with electronic health record systems is a plus
