We’re seeking a compassionate and driven Physical Therapist to provide personalized, one‑on‑one care in patients’ homes—helping them regain strength, mobility, and independence.

What a Transitions at Home Physical Therapist does An Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence in accordance with physician orders and a plan of care under the direction and supervision of the Director of Clinical Services / Clinical Manager.
Hours
8:00am - 4:30pm
Monday through Friday
No holidays

Qualifications / Job Requirements
Active Wisconsin Physical Therapist License
Two (2) years’ experience preferred
CPR Certification
A valid Wisconsin Driver’s License and Car Insurance
