Overview El Hogar Community Services is seeking Mental Health Service Coordinators to meet the needs of our community mental health programs: CORE-Mariposa (South), CORE-Marigold (North), Community Justice Support Program (CJSP), and Sierra Elder Wellness Program (SEWP). Each program is unique in its design and the population they serve. CORE-Mariposa serves adults 18 and older, CJSP serves adults in the criminal justice system, and SEWP serves adults 55+ who require a higher level of care. All programs are located in different locations throughout Sacramento County.

Summary The Mental Health Service Coordinator is responsible for delivering mental health, substance abuse, and support services consistent with the principles of the Psychosocial Rehabilitation Model to an adult population with mental illness. Responsibilities include assessments, service plan development, case management, housing support, skills training, group facilitation, crisis intervention, and individual/group therapy. The coordinator may carry a caseload of 75+ clients in the field and maintain contact as defined by the Program Contract(s).

Responsibilities

Prepares assessments, progress notes, and other clinical documents in keeping with Agency standards and HIPAA regulations.

Conducts assessments, service plan development, housing plan development, case management, psychosocial rehabilitation, crisis intervention, program transfers, and discharges.

Ensures consumers assigned to present caseload are within County Authorization.

Generates appropriate electronic chart notes of all consumer services in the County of Sacramento and Agency guidelines.

Provides group counseling and co-occurring skills groups with clients.

Provides psychosocial rehabilitation and crisis intervention.

Conducts individual psychotherapy.

Participates in internal utilization review and Agency quality assurance reviews and QA subcommittee.

Actively participates positively during internal staff meetings.

Carries a client caseload and consistently meets established productivity requirements.

Assesses consumers for homelessness, risk of homelessness, or other identified housing needs.

Provides necessary linkage to resources to address areas that may affect the client’s ability to obtain or maintain housing.

Completes and submits high‑quality case plans and assessments with minimal supervisory oversight.

Accurately and comprehensively diagnoses consumer symptoms using DSM‑V and is available for diagnostic consultation with non‑LPHA peers.

Actively contributes positively during El Hogar‑provided clinical supervision.

Supervisory Responsibilities This position has no supervisory responsibilities.

Minimum Qualifications

High School diploma or equivalent (Required) and two years or more full‑time direct mental health experience.

Associate degree and six years or more full‑time direct mental health field experience, with at least two of the years post‑Associate degree.

Bachelor’s degree and four years of full‑time direct mental health field experience.

Experience in the Adult Behavioral Health field (preferred).

Computer Skills

Contact Management systems.

Typing speed of 35–45 words per minute.

Database software, Internet software, Spreadsheet software, and Word Processing software.

Ability to assimilate the County’s rapidly required billing software (Smartcare).

Certificates, Licenses, Registrations

Current valid California Driver’s License and acceptable driving record; proof of personal auto insurance (Required).

Current valid CPR training (Preferred).

Compensation

MHA I – $19.14 – High School diploma & two years but less than four years of full‑time/equivalent (FTE) experience in a mental health or related field providing direct mental health.

MHA II – $20.00 – High School diploma & two years of FTE in a mental health‑related field providing direct mental health.

MHA III – $21.50 – High School diploma & four years of FTE direct care experience in the Mental Health field or two years of FTE direct care experience in a mental health‑related field providing mental health; plus required coursework.

MHRS – $23.00 – Master’s degree & 2 years of direct mental health care; Bachelor’s degree & 4 years, Associate Degree & 6 years of direct mental health care.
