The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Primary Responsibilities:
Conducting participant assessments: Perform in-person evaluations to determine the appropriate level of care, responding to emergent, urgent, or non-urgent needs.
Providing direct nursing care: Administer wound care, IV hydration, lab draws, and other nursing tasks during home visits, ensuring the best outcomes for participants.
Coordinating care: Collaborate with physicians, family members, and multidisciplinary teams to create a holistic care plan that addresses all aspects of the participant’s health.
Responding to communications: Answer calls from primary care providers and other staff promptly, triaging concerns as needed and documenting all interactions.
Emergency response and follow-up care: Take immediate action in emergency situations and provide ongoing support for participants.
Participating in quality improvement: Contribute to continuous improvement efforts by providing feedback and participating in staff development and training initiatives.
Qualifications:
Nursing License: Must be a graduate of an accredited School of Nursing and currently licensed to practice in Michigan.
Experience: At least 1-2 years of home care nursing experience (5 years preferred), including experience with frail or elderly populations.
Skills: Strong clinical, communication, and critical thinking skills. Proficiency with Microsoft Word and Outlook is required.
CPR Certification: Current certification (or willingness to obtain it through Senior Care Partners PACE).
Personal Attributes: Ability to work independently, manage stressful situations, and provide excellent customer service to participants and their families.
Additional Requirements:
Reliable transportation with a valid Michigan driver’s license and current car insurance.
Must be fully vaccinated (or willing to be vaccinated) against common communicable diseases, including COVID-19, Hepatitis B, MMR, and others.
