Responsibilities
Perform psychologic/physiologic assessments that integrate changing data.
Formulate a plan of care based on trends of similar patients, including patient education and discharge planning; tailor and prioritize care to individual needs, including cultural, ethical, spiritual needs.
Participate in planning routine transitional health‑care needs (treatment options, patient placement, end‑of‑life options).
Respond promptly to changes in patient needs and provide holistic care using independent clinical judgment.
Adapt educational programs and information to individual patients and families, modifying teaching strategies or content; integrate education during care delivery; collaborate to identify realistic desired outcomes.
Ensure efficiency in care delivery: determine immediate priorities and offer solutions to problems.
Advocate for patient rights, identify potential conflicts, and seek counsel for resolution.
Identify variances from expected outcomes and evaluate patient outcomes to revise the plan of care.
Document all patient care activities per documentation standards; assist the healthcare team with the documentation process.
Consistently communicate and collaborate with health‑care team members, patients, and families to maximize outcomes.
Appropriately delegate patient‑care activities to other team members and participate in staff assignment revisions.
Maintain knowledge of unit initiatives and incorporate outcomes of team/committee work into practice.
Participate in unit teams, work groups, and/or committees; anticipate needs and assist others voluntarily.
Demonstrate knowledge of unit‑based quality improvement initiatives and participate in improvement initiatives using standards, guidelines, and pathways.
Incorporate evidence‑based practice and research findings into nursing practice to enhance outcomes.
Act as a clinical resource and mentor to health‑care team members; display leadership behaviors.
Identify own learning needs based on self‑evaluation and progress toward previous year's goals.
Accountable for all documentation and completion of required continuing education and competencies.
Ensure practice improvements by assuming responsibility for lifelong learning.
Provide direction and guidance regarding practice; serve as resource, preceptor, and mentor; exhibit leadership skills in critical thinking, decision‑making, and problem solving.
Consistently perform 12 organizational service standards focused on values, service, and quality.
Qualifications
2 years of acute‑care nursing experience.
Graduate of an accredited school of nursing.
Current Louisiana State license or temporary license as RN.
BLS certification.
Advanced Cardiac Life Support for the specialty within six months.
