Job Summary The Pharmacist is responsible for appropriate, evidence-based medication use for the institution. They ensure patients get the right medication at the right dose. Pharmacists must have a medication safety viewpoint and be in compliance with all applicable standards and regulations (TJC, BOP, CMS, etc.). Pharmacists oversee the activities of pharmacy technicians, and must engage in teaching as part of the hospital's mission.
Core Responsibilities and Essential Functions
Provides clinical pharmaceutical care to patients. Prevents, detects, and resolves medication-related problems and makes appropriate evidence-based, patient-centered medication therapy recommendations.
Participates as an effective member of interdisciplinary care team(s) and communicates follow-up with team.
Effectively verifies patient medication orders for appropriateness and accuracy.
Provides effective medication education to patients, families, team.
Enhances the quality of delivery of care by actively participating in the development and implementation of performance/quality improvement and patient safety initiatives.
Represents pharmacy services on institutional/practice area committees, quality unit councils.
Conducts monthly unit inspections of assigned area.
Demonstrates proficiency in pharmacy distributive services, ensuring quality, safety, and efficiency.
Provides oversight for pharmacy technicians and collaborates to improve distribution quality and efficiency.
Supports the education and professional development of the Pharmacy Department and other hospital disciplines; precepts residents, students, or technicians as assigned.
Engages in professional self-development, completing competency requirements by deadlines and attending continuing education.
Performs other duties as assigned.
Complies with all Wellstar Health System policies, standards of work, and code of conduct.
Required Minimum Education
Bachelor of Pharmacy
Doctorate Pharmacy (preferred)
Required Minimum License(s) and Certification(s)
Registered Pharmacist
Additional License(s) and Certification(s) All certifications are required upon hire unless otherwise stated.
Required Minimum Experience Minimum 2 years in Pharmacy as a pharmacy technician, intern and/or pharmacist (preferred).
