Senior Solutions Vermont Pay Range This range is provided by Senior Solutions Vermont. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more.
Base pay: $22.00/hr - $24.00/hr
Qualifications Bachelor’s degree in human services and prior case management experience preferred. A combination of relevant education and experience may be substituted for a degree. Must have reliable transportation and a valid driver's license.
Responsibilities
Collaborating with other community partners to create a person-centered plan of care.
Monitoring and reassessing program service plans as required.
Having a working knowledge of services and programs available to older Vermonters and people with disabilities.
Assisting, when necessary, to gain access to existing services such as public benefits, housing, legal services, etc.
Ensuring that all provided services do not exceed budgeted amounts and are consistent with DAIL guidelines.
