Registered Nurse / ACT Team / Community (Aiken)

This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Aiken-Barnwell Mental Health Center, 1135 Gregg Highway, Aiken, SC 29801. Services are delivered in the community and patients' homes.
Responsibilities for the Registered Nurse / ACT Team / Community (Aiken)
Provide direct services to ACT patients to include but not limited to: assessing the health, social, psychiatric and medical needs of Center patients; administering scheduled intramuscular medications to patients as ordered by prescribers; monitoring patients regularly for beneficial effects, side effects, and other medication related issues; and developing strategies to maximize taking medications as prescribed.
Engage in health promotion, prevention and education activities.
Ensure patients are seen in accordance with their needs (medical/psychiatric appointments).
Complete clinical documentation using collaborative documentation and ensures services are documented in accordance with OMH/QA requirements.
Participate in clinical supervision and daily treatment team meetings.
Comply with Corporate Compliance, Policy and Risk Management Plan.
Ensure timely completion of prior authorizations.
Pursue professional, development through collegial consultation, continuing education and licensure.
Perform other duties as needed.
Qualifications
Graduation from an accredited school of nursing.
Licensed as a registered nurse by the SC Board of Nursing.
One-year experience working with adults diagnosed with serious mental illness and working knowledge of psychiatric medications.
Additional Requirements
One-year experience working with adults diagnosed with serious mental illness and working knowledge of psychiatric medications.
Ability to lift 5 pounds and operate a state vehicle.
Bilingual abilities in English and Spanish (or another language) are a plus.
Post hire, employees must:
Have knowledge of the techniques and best practices of the nursing professional.
Have knowledge of medical and nursing terminology.
Have the ability to evaluate a client's medical status and needs and use knowledge and judgment to respond appropriately.
Have the ability to plan, implement and evaluate client medical care plans.
Have the ability to provide individual health education and counseling.
Have the ability to communicate effectively both orally and in writing.
Have the ability to function as a team member.
Have knowledge of privacy laws.
Have a high degree of independence and self-motivation is expected.
Participate in Employee's activities that are governed by SCDMH and Center policies and procedures.
Position is essential and may be called back during crises/disasters/emergencies.
Position will be part of the Assertive Community Treatment Team.
Requires flexible schedule.
Services are delivered in the community and patients' homes.
Travel in Aiken and Barnwell counties required.
