LPN Licensed Practical Nurse (EVENING SHIFT)
Location: Mystic, CT
Shift: Evening (2:30pm – 8:00pm)
Hours: 22 hrs/week, End of Week
Pay: $27.00/hr – $37.00/hr
Sign‑on bonus: $1,000.00
Responsibilities
Collect data based on age, needs and abilities in an ongoing and systematic manner and participate in regular care planning meetings.
Communicate and collaborate effectively with residents, families, colleagues and other members of the health care team to meet resident needs.
Provide education, supervision and counseling to resident and family regarding assisted living procedures and other care needs.
Document accurate, pertinent and comprehensive information in a timely and concise manner in the client service record.
Conduct supervisory visits of resident care assistants, companions and homemakers as required.
Participate in educational programs, in‑services, meetings and workshops and utilize current literature to enhance professional skills and knowledge.
Under supervision of SALSA/RN DESIGNEE and in collaboration with physicians, pharmacists and outside agencies, oversee medication management including ordering, storing, reconciling and preparing medications for administration or CNA prompt.
Perform other duties as assigned.
Qualifications
Education: Graduate of an accredited School of Nursing.
Experience: 1–2 years of clinical experience in a hospital or similar health care setting preferred.
Certificates, Licenses, Registrations: Current LPN license; CPR certification registration.
