The Opportunity RIPCPC is seeking a Patient Navigator to support patients across its 65 primary care practices throughout Rhode Island. The role focuses on post‑visit and behavioral health interventions, complementing the activities of embedded office staff and other centrally employed team members. The Patient Navigator plays a key role in coordinating care, facilitating referrals, and improving patient access and continuity of care.

What you’ll do

Act upon referrals ordered during office visits and facilitate appointment scheduling and referral entry in insurance‑based systems, prioritizing BCBS and UHC plans.

Ensure accurate and thorough documentation of referrals within the EMR at every step of the referral process from initiation to closure, including closing the loop.

Ensure documentation is collected from patient specialist visits.

Identify SDOH needs through coordination of services and handoff to care team members where appropriate.

Schedule appointments internally and externally with outside specialists.

Facilitate referrals between care team members.

Act upon behavioral health referrals ordered during office visits and facilitate appointment scheduling.

Collaborate with RIPCPC clinicians for timely behavioral health referrals and schedule appointments via the RIPCPC booking site.

Facilitate relationships with community organizations.

Field calls for patients seeking a new primary care physician and provide contact information for RIPCPC providers accepting new patients.

Directly schedule new patients via Epic for select offices.

Facilitate Uber Health rides to patients’ primary care offices via the Uber Health Portal.

Attend and participate in related meetings and perform other job‑related duties as assigned.

Who you are

2–3 years in the medical field, preferably with a Medical Assistant or Quality Assistant background.

Experience with patient‑facing roles, particularly one involving telephonic outreach.

Experience in primary care practices.

Experience with value‑based metrics around quality and cost containment.

Experience with EHRs, particularly Epic, eClinicalWorks, and Athena.

Comfortable adapting to changing day‑to‑day priorities and assignments.

Comfortable working in a team.

Excellent verbal communication and interpersonal skills.

Detail‑oriented, self‑motivated, and comfortable in ambiguity.

Proficiency in using the Microsoft Office Suite.

Bachelor’s degree in a healthcare‑related field (preferred).

This is an hourly position with a range of $20–$23 per hour. Equivalent annual salary range: $41,600 – $47,840 USD.
