This position includes a $3,000 sign‑on bonus. The Case Manager in the Children’s Outpatient Program (COP) is responsible for providing assessment, linking, coordinating and monitoring family education and support, transportation, treatment planning and follow‑up services to children and families. Services are delivered in the office and in the community based on the needs of the client, family. The Case Manager coordinates and integrates services provided to the family both internally and externally. Working hours are Monday through Friday 8:30 – 5:00 with an additional 4 hours after 5:00 required each week. Weekend work may be required due to program and consumer needs.

Responsibilities

Conduct case management assessments and other clinical assessments necessary to identify areas of need and service preferences.

Gather and integrate information from all available sources, including client self‑reports, family members, guardians, probation officers, medical and treatment history.

Assist with the development of comprehensive Person‑Centered Treatment Planning with consumers and other members of the treatment team (consistent with Case Management Assessments and current Medicaid Guidelines).

Provide face‑to‑face interventions, both in the office and in the community, to each assigned consumer at least monthly.

Maintain monthly contact with treatment team members and other providers to monitor and coordinate service delivery efforts.

Assist clients in determining their eligibility for entitlements, community services and supports.

Work in concert with other internal and external service and support entities.

Develop resources and connections with community resources.

Link clients to community services and supports based on the needs and preferences identified in their assessments.

Coordinate with client’s school, primary and other health‑care providers to assure continuity of care.

Conduct skill‑building interventions with individual consumers and groups of clients as indicated in their Person‑Centered Treatment Plan.

Conduct home visits and community outreach as needed to help engage clients, identify and address needs and barriers.

Conduct crisis planning with clients.

Complete all required documentation per policy and procedure.

Maintain appropriate state licensure and certifications.

Develop a schedule with supervisor and work at least 4 hours each week after 5:00.

Perform other duties as assigned.

Requirements

Bachelor’s degree in social work, psychology, or a related human services field from an accredited social services program, with a 60‑day training period beginning on the date of hire.

Candidates must obtain the Children’s Mental Health Professional credential within 120 days of hire.

Coursework, fieldwork or job experience demonstrating competence in collaborating with clients and the community, cultural competence, good organizational skills, and the ability to work independently and as a team member.

Three (3) years of experience in the examination, evaluation and treatment of minors and their families, or a master’s degree plus one (1) year of experience in the same area.

Valid chauffeur’s license and a favorable driving record. Candidates without a license may apply, but must acquire one within 30 days of employment.

Maintain valid state licensure and certifications.
