Registered Nurse (RN) – Part‑Time
STGi is seeking a Registered Nurse to provide services at our Community Based Outpatient Clinic (CBOC) in Auburn, New York.
Job Summary The RN Care Manager represents veteran patients in all clinic areas, coordinates care with the VA Medical Center and stakeholders, and manages patient care under the direction of the CBOC.
Essential Functions
Provide nursing care based on orders from the physician, mid‑level provider and CBOC Clinic Manager.
Supervise support staff in delivery of care.
Lead and participate in PACT team huddles.
Work with the Primary Care Physician to ensure adequate staffing for quality care delivery.
Maintain universal precautions and infection control practices.
Supervise medical assistants and/or licensed practical nurses as they obtain initial patient information.
Prepare patients for examination and assist the PACT Provider upon request.
Ensure smooth and timely flow of patients in accordance with VA access standards and triage protocol.
Demonstrate and perform triage activities for all walk‑in patients and medical phone inquiries.
Administer medications per facility protocols.
Perform CPR and assist during respiratory and cardiac arrest procedures.
Perform a range of nursing procedures, including dressing changes, suture removal, oxygen administration, wound irrigations, catheterizations and venipunctures.
Prepare patients for laboratory procedures by providing information and instructions.
Set up and operate therapeutic and emergency equipment.
Document all pertinent patient information in VISTA and veteran health information systems.
Keep the VISTA clinic reminder tracking system current for each enrolled patient.
Assist in scheduling patients for initial and follow‑up appointments in accordance with VA access standards.
Keep patients informed about their plan of care, including lab and diagnostic test results as needed.
Provide health materials, resources and education to patients and families.
Assist providers in ensuring all patient information on electronic encounter documentation is complete and accurate.
Participate in ongoing performance improvement program between subcontractor(s) and the VAMC.
Ensure all required reports, clinical and administrative documentation is completed in an accurate and timely fashion.
Participate in management of day‑to‑day CBOC operations.
Comply with all federal, state, local, Joint Commission OSHA, Veteran Affairs, subcontractor safety and operational regulations, directives and standards.
Maintain confidentiality of all information and support patients’ privacy, rights, and safety.
Perform duties of the CBOC Clinic Manager when absent (if applicable).
Perform other work‑related duties as assigned.
Required Experience and Skills
Three years of experience in medical surgical unit, outpatient facility, primary care and/or adult health preferred.
Current and unrestricted nursing license in New York.
Current CPR/BLS from American Heart Association or Military Training Network.
Knowledge of and ability to apply professional nursing principles, procedures and techniques.
Ability to assess patient condition, recognize adverse signs and symptoms, and react swiftly in emergency situations.
Ability to set realistic, measurable education goals for patients.
Ability to develop and provide individualized and group counseling, guidance and health instruction focused on disease prevention.
Ability to teach patients and families proper home health care activities.
Knowledge of pharmacological agents used in patient treatment.
Exceptional verbal, written and telephone communication skills.
Knowledge of Microsoft Office, electronic medical records and computer maneuverability.
Must be detail oriented and able to multi‑task.
Required Education
RN graduate of a school of professional nursing approved by a state‑accrediting agency (ACEN or CCNE).
Bachelor of Science in Nursing (BSN) preferred.
Working Conditions / Physical Requirements
Moderate physical activity; handle average‑weight objects up to 25 pounds; stand or walk >4 hours per day.
Typical interior office work environment.
Respond to any medical emergency within the clinic; fast walk or jog may be necessary.
