We’re seeking a Licensed Social Worker (LSW) to provide psychosocial support and care coordination in our Emergency Department at Loyola Medicine. This part‑time, 12‑hour night shift role requires an LSW to collaborate with a multidisciplinary team to assess, intervene, evaluate, and educate patients and families. Employment Type

Provide psychosocial assessment, intervention, evaluation, consultation, and education to patients and families as part of comprehensive care management services. Identify high‑risk patients from a psychosocial/financial perspective and provide information, support, counseling, care management, and referrals to appropriate resources. Serve as liaison for complex transitions/discharges between the hospital and community agencies to exchange clinical referral information. Contribute to training of social work professionals as part of the education program at LUHS. Coordinate details of placement and actual discharge of complex patients to appropriate agencies; discuss discharge plans and barriers with interdisciplinary team. What You Will Need

Master’s degree in Social Work. Licensed Social Worker (LSW) required; LCSW preferred. Excellent communication skills. Ability to manage crisis situations calmly and effectively. Ability to work under stressful conditions and in difficult situations. Salary Range: $30.00 – $41.59 per hour.
