About the Role The Women’s Health Navigator at SMJH will act as the clinical lead for the LDRP and liaison with clinicians, nursing staff, nurse midwives and administration. Will assist in care coordination for patients requiring additional resources. This position does require some flexibility for evening hours for support groups and trainings within the Charlottesville, VA area.
The Women’s Health Navigator is the entry access for women across the life span for Sentara Martha Jefferson Hospital which includes inpatient, outpatient and community services. Daily activities typically account for 60% of the Navigator’s time and include those duties required to ensure the ongoing operational and managerial elements for the program to run smoothly. These daily activities may include patient and family relations, physician relations, quality patient care through physical assessment, preventative health care, case management, consultation, and referrals.
Program development activities include the activities necessary to ensure the long‑term success and growth of the program. Program development activities typically account for 40% of the Navigator’s time and include patient education, program development, patient satisfaction, continuous improvement and data collection, research and marketing. This integral position is designed to decrease fragmentation of care and coordinate services. The navigator creates a seamless flow through the health care system for the patient and family with family‑centered care as the focus.
LDRP Sentara Martha Jefferson’s Women’s Health includes L&D and a 24‑bed post‑partum mother/baby unit. Our Level 2 newborn specialty care nursery has a 1‑to‑1 ratio while our intermediate‑care babies have a 1‑to‑3 nursing ratio. Our LDRP unit also offers post‑operative care for complex GYN and Breast surgical needs. This specialty area has a 100‑member team cross‑trained to offer best‑in‑class health care to Charlottesville, VA, and surrounding areas. The schedule is primarily 12‑hour shifts with rotating weekends and holiday requirements. Experience
3 years of acute‑care registered nurse experience in Women’s health – Required.
Education or navigation experience preferred.
Strong understanding of patient safety protocols.
Manages complex cases in fast‑paced environments.
Communication with patients and providers.
