We bring deep expertise in physical therapy, occupational therapy, speech therapy, wound care, and complex care management. Responsibilities

Admits patients eligible for home care within 24–48 hours under physician’s order and completes all required documentation per agency policy Performs comprehensive assessments to identify patient needs, establish goals, and develop individualized care plans Provides skilled nursing services including evaluation, procedures, teaching, and training as outlined in the Plan of Care Updates care plans and resolves patient problems as conditions change or at recertification Communicates patient status and interdisciplinary needs to Clinical Supervisor, referring physician, and follow-up clinician Submits documentation including visit notes, communication logs, change orders, and supervisory notes per agency schedule Conducts OASIS assessments and submits recertification paperwork in compliance with Medicare and agency standards Supervises Home Health Aides and documents per Medicare and agency requirements Participates in team activities including staff meetings, educational in-services, discharge planning, and process improvement initiatives Serves as a preceptor for new nursing staff during orientation as requested Delivers Age-Friendly Care by incorporating the 4Ms framework: What Matters, Medications, Mentation, and Mobility Meets physical demands of the role including lifting up to 40 pounds, driving 2–4 hours daily, and performing repetitive movements Qualifications

Valid driver’s license with a good driving record Sound professional clinical judgment and ability to assess and document patient needs Proficient clinical skills and ability to formulate individualized patient care plans Excellent verbal and written communication skills Proficiency in PC use, including e‑mail, clinical, word processing, spreadsheet and presentation software Active CPR Certification Preferred Knowledge, Skills and Experience

One year of experience as a home care professional nurse and competence in comprehensive home care assessment Perks and Benefits
