Position Summary
Assists the Infectious Disease team in the provision of quality services. Assists clients living with HIV in obtaining access to needed prescription medications via Pharmaceutical Assistance Programs, Georgia AIDS Drug Assistance Program, and assists them with enrolling in Georgia’s Health Insurance Continuation Program. Provides data on enrollments once qualified, to assist RW team in tracking enrollment efforts and results obtained. Cross‑trained in intake procedures and CAREWare/e2Fulton data entry as backup. Assures completion of clinical quality measures, specific to the patients’ targeted diagnoses, and assists in the development, implementation and monitoring of quality management projects (Plan‑Do‑Study‑Act) to improve underperforming outcomes.
Duties and Responsibilities
Obtains from patient interview, or extracts from Electronic Health Record (EHR) the necessary eligibility information to assist patients in enrolling into pharmaceutical companies patient assistance programs (PAPs) and Georgia AIDS Drug Assistance Program (ADAP) to obtain needed medications, and Health Insurance Continuation Program (HICP). May receive and scan required eligibility documents at intake and recertification, as well as enter data on units of services provided to RW clients into CAREWare/e2Fulton
Receives lists of identified patients needing supports to reduce “social determinants of health” which may be impeding improved health and “guides and links” those patients through the healthcare system. This includes helping patients arrive at scheduled appointments on time and prepared and following up with patients that have not attended appointments.
Identifies and develops relationships with personnel in departments involved in care of patients (i.e. physicians, pharmacy, nursing, social service and mental health staff, etc); offers educational sessions to inform practitioners of the role and services and to encourage referrals. Identifies and develops and maintains relationships with other referral sources, such as medical clinics and allied healthcare providers to encourage and facilitate referrals. Liaises with pharmaceutical patient assistance programs, ADAP and HICP representatives.
Assistswith databases/data systems for Ryan White Program to collect and manage client‑level (CLD) and aggregate services and outcomes data in CAREWare/ e2Fulton. Assist in analyzing client‑level utilization data (CLD) for accuracy and consistency with quarterly reports
Compares CLD to annual HIV epidemiology report from the Georgia Department of Health and analyzes for utilization trends.
Prepare service‑specific custom data reports as needed by the Ryan White Program Manager, fiscal staff, and Fulton County RW Program staff.
Provides technical assistance to team members to support their appropriate documentation of services and outcomes.
Assists and provides technical assistance to multi‑disciplinary team members in understanding data management processes and issues that arise in analysis and reporting to ensure accurate and complete data to meet planning, grant reporting, performance and quality management requirements..
Obtains monthly EHR quality and productivity reports from informatics and transaction reports from the revenue manager
Effectively communicate data to the Ryan White team members and providers.
Serves on Quality Management Committee of the Ryan White Planning Council
Reviews clinical outcomes data to assess program progress on target objectives outlined within the specific Part A Implementation Plan.
Assists with developing, implementing and monitoring quality management projects (Plan‑Do‑Study‑Act) and processes to improve underperforming outcomes, or develop new measures as needed to support ongoing program planning. Shares reports of progress through monthly Quality Improvement team meetings.
Develop and deliver semi‑annual Service Area Reviews that incorporate results of client‑level outcomes evaluation and utilization data for use by SMC in its prioritization and allocation process.
Assist in preparation of the annual Part A grant applications and in meeting Ryan White grant conditions of award.
Upholds guidelines set forth in clinic/organization, including maintenance of patient confidentiality.
Participates in ongoing training to maintain core proficiencies of the program or service, meetings and committees as necessary.
Performs other duties as necessary to cover Ryan White program needs and meet deadlines. Completes special projects as requested.
Knowledge, Skills and Abilities
Ability to translate services recorded within electronic health records into required data elements and services information into CAREWare/e2Fulton .
Ability to communicate with sensitivity to needs and concerns of patients living with HIV.
Excellent documentation skills, with ability to check data for errors and assure accuracy within required reports.
Ability to provide technical guidance and direction to others in support of accurate data utilization and reporting.
Ability to ensure adherence to all applicable policies and procedures.
Knowledge of medical terminology.
Minimum Qualifications Either:
Masters degree in health or human/social services field with coursework in program planning, evaluation, statistics, and/or quality management PLUS a minimum of two years of experience in public or community health service.
Or:
Bachelor’s degree in health or human/social services field with coursework in program planning, evaluation, statistics and/or quality management PLUS a minimum of four years of experience in public or community health service.
Experience in data input, data analysis, and electronic health record systems. Prefer prior experience in grant preparation and administration.
Typical Physical Demands Requires manual dexterity and eye‑hand coordination sufficient to operate a keyboard, photocopies, telephone, calculator, and other office equipment. Requires normal range of vision and hearing. May require sitting for prolonged periods of time during patient appointments and documentation.
Involves physically assisting patients, occasionally lifting and carrying items weighing up to 20 pounds.
Typical Working Conditions Administrative and medical office environment with potential for exposure to communicable diseases, bodily fluids, toxic and other substances common to clinic environment.
