Occupational Therapist (Home Health – Part‑Time or Per‑Diem)

Hayward/Oakland/San Leandro – Alameda County, California
Benefits/Perks:
Pay range: $110–$150 per diem visit/evaluation/SOC (rate varies by coverage area, experience, and number of visits/week)

Job Summary:
We are seeking an Occupational Therapist to join our team. The ideal candidate works well independently and within a team, maintains a positive attitude to encourage patients in recovery, and has knowledge of physical therapy guidelines and recommendations.
Responsibilities
Create a Plan of Care by evaluating and assessing patients’ functionality
Assist patients with exercises or daily functional tasks by correcting form, motivating, and ensuring safety
Maintain accurate records for all patients, treatments, and progress
Maintain all equipment, ensuring a clean and safe work environment
Qualifications
Current state licensing for Occupational Therapy
CPR certification required
