BrightStar Care of Acton–Andover–Lowell is seeking Per Diem Physical Therapists (PT/RPT) and Occupational Therapists (OT) to complete assessment-only visits for skilled home health patients. This role is ideal for clinicians looking for flexible scheduling, meaningful one-on-one patient care, and limited ongoing treatment responsibilities. Key Responsibilities
Conduct initial and follow-up assessments in accordance with physician orders and clinical guidelines Evaluate patient functional abilities and determine care needs Establish plan of care and provide appropriate clinical recommendations Educate patients and caregivers on safety, mobility, and home exercise strategies Complete timely and accurate documentation in compliance with state and regulatory standards Collaborate with nursing and care teams to support optimal patient outcomes Qualifications
Graduate of an accredited Physical Therapy or Occupational Therapy program Active MA PT or OT license (or license-eligible) Experience in home health, skilled nursing, or sub-acute rehab preferred, not required Strong clinical judgment, documentation accuracy, and communication skills Ability to work independently and travel within the local service area Schedule & Compensation
