Sun River Health is looking to hire an accomplished Full‑time Primary Care Physician (IM/FM) to join our team in Wyandanch, NY. The Primary Care Physician is responsible for managing a diverse patient panel with multiple co‑morbidities in a busy Federally Qualified Health Center. Care is delivered in collaboration with advanced practice providers, on‑site support staff, with an emphasis on preventative health and quality patient care.
Responsibilities
Participate in planning and developing health care services, treatment of conditions and illnesses within scope.
Diagnose and provide treatment plans for patients.
Collaborate and supervise advanced provider clinicians.
Complete and participate in chart review process.
Referral and follow‑up of patients when needed.
Collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction.
Clinical Supervision: Oversee the clinical work of students or new providers, ensuring they adhere to best practices and organizational standards.
Mentorship and Guidance: Provide mentorship and guidance to students or new providers, helping them develop their clinical skills and professional behavior.
Educational Support: Facilitate educational sessions, workshops or seminars to enhance the learning experience of the mentees.
Performance Evaluation: Assess the performance of students or new providers, provide constructive feedback and document their progress.
Patient Care: Ensure high‑quality patient care is maintained while supervising and mentoring students or new providers.
Collaboration: Work closely with academic institutions and other healthcare professionals to coordinate the educational activities and ensure a comprehensive learning experience.
Minimum Education Requirement
Doctorate Degree (MD, DO)
Licensure / Certification / Registration
Active/Valid license as an MD or DO in New York State, DEA, Board certification
Specialized Skills & Knowledge
Bilingual in English and Spanish (preferred)
Experience in HIV care and MAT (Medication Assisted Treatment) (preferred)
Demonstrated ability to work with diverse patient population in community health setting.
