As the Palliative Care Nurse, you will provide direct patient and family care through palliative care consultations, ensuring specialty services for patients with serious illness and a prognosis of one year or less who are not ready or eligible for hospice care.
Primary Responsibilities
Demonstrate effective and ethical use of approved evidence‑based palliative medicine practices across all patient care domains.
Incorporate palliative care body of knowledge to formulate an effective plan of care.
Systematically gather pertinent information from medical records, family history, caregivers, and conduct comprehensive physical, emotional, psychosocial, and spiritual assessments.
Complete a comprehensive assessment using the palliative care domains, interpret findings, conduct medication reconciliation, formulate problem‑related diagnoses, initiate a therapeutic plan, and evaluate outcomes.
ACHPN certification encouraged after 2 years of employment or when experience requirements are met.

Required Qualifications
RN/APRN licensure in state of practice.
Nurse Practitioner/CNS with prescriptive authority.
BCLS licensure.
Current CPR certification.
5+ years of clinical nursing experience.
Preferred Qualifications
1+ years as APRN in Hospice, Palliative Care, or Oncology.
Advanced practice palliative care certification (ACHPN).
Compensation for this specialty generally ranges from
$104,500
to
$156,000 . Total cash compensation includes base pay and bonus, and is based on local labor markets, education, work experience and may increase over time based on productivity and performance. We comply with all minimum wage laws as applicable.
In addition to salary, benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401(k) contribution (subject to eligibility requirements).
