Restorative Nurse
ALIYA Healthcare Consulting is seeking a compassionate and experienced Restorative Nurse to join our dedicated team at Ryze at Homewood.
Compensation Base pay range: $38.00/hr - $45.00/hr
Responsibilities
Provide quality nursing care by assessing patient health problems and needs, developing and implementing nursing care plans, and coordinating restorative programming and documentation to promote quality outcomes for residents.
Supervise and educate restorative staff and nurse’s aides in restorative care; complete restorative nursing documentation in the EHR, including ADL daily documentation, restorative nursing programs, and monthly/quarterly progress notes.
Communicate effectively with patients, families, and interdisciplinary team members.
Lead coordination of care for restorative programs and assist team members as directed by management.
Coordinate with other departments to enhance patient care and provide input on quality improvement initiatives.
Address customer service: greet patients, anticipate needs and resolve issues, maintain a positive attitude, and strive for high satisfaction.
Use organization policies and procedures, and keep documentation accurate and thorough.
Qualifications
Licensed Registered Nurse (RN) in the State of Illinois, in good standing.
Minimum 1 year experience in long‑term care or hospital setting (preferred).
Critical thinker capable of making concrete clinical decisions.
General knowledge of short‑term rehabilitation goals and objectives.
Ability to educate families and patients on diagnosis, medications, and clinical services.
CPR certified.
Basic understanding of therapy equipment and modalities.
Thorough knowledge of Restorative Nursing and MDS (preferred).
Excellent written and verbal communication skills.
Strong interpersonal skills, organized, and able to prioritize work.
Patient‑care focus with excellent customer‑service orientation.
