Location: St Augustine, FL
Base pay range $56.00/hr - $65.00/hr
Your Role As a Physical Therapist
Travel to patients’ homes to provide personalized physical therapy services that support recovery and independence.
Conduct thorough assessments to evaluate each patient’s condition and rehabilitation needs.
Develop and implement goal‑oriented treatment plans in collaboration with patients and caregivers.
Guide and supervise Physical Therapy Assistants to ensure high‑quality care.
Qualifications
Unencumbered Florida Physical Therapy License (required).
Valid driver’s license, auto insurance, and reliable transportation (required).
Experience with OASIS documentation (preferred).
CPR Certification (preferred).
Ability to delegate and supervise Physical Therapy Assistants (preferred).
