Purpose and Scope Functions as part of the hemodialysis health care team as a Registered Nurse applicant under the direction of a Staff Registered Nurse. Provides quality patient care daily in line with FMS policies, procedures, and training. Supports the FMCNA mission, vision, values, and customer service philosophy, and participates in Quality Enhancement Program (QEP) and continuous quality improvement (CQI) activities. Adheres to compliance and administrative policies. Employees must obtain nursing license and registration within the timeframe outlined in state regulations or within 90 days from hire.

Principal Duties and Responsibilities

Staff Related

Directs Patient Care Technician’s provision of safe and effective chronic hemodialysis therapy in compliance with facility policies and regulations.

Delegates tasks to all direct patient care staff, including LVN/LPNs, Patient Care Technicians, and Dialysis Assistants.

Assesses daily patient care needs and develops appropriate care assignments.

Monitors staff for technique compliance and policy adherence.

Participates in staff training, orientation, and required meetings.

Patient Related: Education

Ensures educational needs of patients and families regarding End Stage Renal Disease (ESRD) are met.

Provides ongoing education on renal disease, vascular access, dialysis therapy, and related health conditions.

Discusses diet/fluid and medication compliance and records related education.

Educates on adequacy measures (OLC, AMP, UKM) and transplant awareness.

Explains laboratory values and dialysis therapy relationships, compliance with schedules, medications, and fluids.

Dialysis Treatment

Provides safe and effective delivery of care to ESRD patients.

Accurately implements treatment prescriptions, including sodium modeling and ultrafiltration.

Assesses patient response, adjusts treatment, and communicates concerns to the team leader or physician.

Initiates Initial and Annual Nursing Assessment and documents ongoing patient needs.

Participates in all phases of patient evaluation, initiation, monitoring, and termination of hemodialysis therapy.

Takes intervention for changes in adequacy status and manages access flow issues identified by OLC/AMP.

Supervises and monitors hemodialysis access care, including interdialytic transfusions and documentation.

Ensures accurate documentation on the Hemodialysis Treatment Sheet.

Laboratory-Related

Reviews, transcribes, and enters physician lab orders accurately.

Prepares lab requisitions and ensures correct tubes and procedures.

Ensures follow‑through on missed or insufficient labs per policy.

Handles DOT packaging and shipment of specimens.

Reports abnormal or urgent results to the appropriate physician.

Forwardes lab results to physicians as requested.

General Duties

Enforces company policies, state, and federal regulations.

Maintains safe, efficient shift operations.

Acts as a resource for staff members.

Meets with the Clinical Manager to discuss personnel and patient care status.

Collaborates with physicians and healthcare team to adjust care.

Assists patients with prescription refills and orders entry.

Oversees all patient documentation and maintains facility drug list.

Maintains competency in emergency procedures and initiates CPR as needed.

Ensures availability of emergency equipment and vaccinations.

Administers prescribed medications and PRN doses with documentation.

Records controlled substances per law and coordinates patient transportation if necessary.

Maintenance/Technical

Maintains clean and sanitary dialysis facility treatment area.

Operates all dialysis equipment safely and effectively.

Ensures patient stations are clean and blood is addressed per bloodborne pathogen policies.

Medical Records & Documentation

Ensures accurate documentation of orders, lab results, vitals, and patient status in the Medical Information System.

Verifies completion of Hemodialysis Treatment Sheets and daily procedures.

Initiates CQI activities and completes monthly reports and nurse progress notes.

Completes patient care plans, transplant status reviews, and annual assessments.

Other

Maintains required credentials and certifications in active status.

Works flexibly in a challenging environment with multitasking and adaptability.

Maintains regular and punctual attendance.

Performs related duties as assigned.

Physical Demands and Working Conditions

Heavy lifting and patient assistance up to 200 lbs; frequent prolonged standing; ability to bend, lift up to 30 lbs, and move equipment.

Work environment includes controlled temperature, moderate noise, and potential exposure to infectious materials.

Requires Ishihara Color Blindness test; accommodations may be considered.

Education

Graduate of an accredited School of Nursing.

Current state Temporary Permit.

Must meet state practice requirements.

Experience and Required Skills

0–1 year of related experience.
