Develop patients’ plans of care in collaboration with physicians, patients, and utilization management professionals. Coordinate case management across the multi‑disciplinary health care team. Initiate, deliver, and evaluate patient‑ and family‑centered health teaching. Strive to achieve optimal health outcomes with compassionate care focused on the patient’s experience. Complete required documentation (visit assessments, OASIS, 485, verbal orders) accurately and promptly. Attend compliance training and adhere to organizational standards, policies, and local, state, and federal regulations. Qualifications

Graduate of an accredited professional nursing school is required; bachelor’s degree preferred. Prior nursing experience preferred. Licensure, Certifications, and Clearances

Current RN license in the state of employment or a state covered by the Nursing Licensure Compact. CPR certification per AHA standards, both didactic and skills demonstration within 30 days of hire. Valid driver’s license and automotive insurance. Compliance with Act 33, Act 34, OAPSA, and other applicable local regulations.
