Position Overview:

We are seeking a skilled Home Health RN Case Manager with a minimum of 1 year of home health experience and proficiency in WellSky Kinnser EMR to join our 4‑star team. The role focuses on preventing hospitalization by providing exceptional patient care in the comfort of patients’ homes. Safe Life utilizes Andy AI to assist clinicians with OASIS completion and submission within 24 hours.

Responsibilities Provide comprehensive patient care, including assessment, care planning, OASIS completion, and collaboration with interdisciplinary teams to ensure high‑quality home health services. Monitor patient progress and adjust care plans as needed.

Qualifications

Valid Illinois RN license

Valid CPR certification

Valid driver’s license

Valid auto insurance

Minimum 1 year of home health experience

Proficiency in WellSky Kinnser EMR

Strong clinical assessment and communication skills

Compassionate and patient‑centered approach
