Primary Care Nurse Practitioner (NP)

full-time and part-time
scheduling options, providing flexibility and balance in a supportive, multidisciplinary environment. The Nurse Practitioner will play an integral role in delivering patient-centered care, focusing on prevention, wellness, and management of chronic and acute conditions in an outpatient setting. Essential Position Responsibilities
Provide comprehensive primary care services, including preventive, acute, and chronic disease management.
Conduct physical exams, order and interpret diagnostic tests, and develop individualized care plans.
Prescribe medications and treatments in accordance with clinical guidelines.
Educate and counsel patients on disease prevention, health maintenance, and lifestyle modification.
Collaborate with physicians, specialists, and other care providers to ensure coordinated, high‑quality care.
Maintain accurate and timely documentation within the electronic health record (EHR).
Participate in care coordination and quality improvement initiatives.
Qualifications
Education:
Graduate of an accredited Nurse Practitioner program (MSN or DNP).
Licensure & Certification:
Active or eligible
Nurse Practitioner license in New York ; national certification required.
Experience:
Prior experience in primary care or family medicine preferred, but new graduates are welcome to apply.
