Trustbridge Hospice, a part of Empath Health , is seeking a caring and motivated RN to join our team in Admissions in the Palm Beach County area as a Lead Admissions RN while providing exceptional care on the following schedule: 9:00am – 7:30pm

What you\'ll Do
Carries primary responsibility for evaluation of all patients referred for hospice services.
Is able to articulate all components of the organization\'s continuum of care and refer patients and families to those as necessary.
Assists referring physicians and/or his/her designated liaison with coordinating a plan of care/treatment for hospice appropriate patients and their families.
Serves as the liaison between our company and physicians, hospitals, long-term care facilities, and other health care professionals.
Actively markets, through education, the hospice concept to all health care professionals and providers, as well as the community at large.
The Lead Admission RN will train, teach, educate, mentor new and identified AN team members who require guidance and support.
Formulate and organize training materials to assist in streamlining admission process and education for AN team members.
Assist in establishing standard, clear, concise CTI presentation application with all AN Team members.
Instrumental in promoting and teaching strong clinical knowledge to AN team, resulting in admission department reaching high patient conversion goals.
What You\'ll Need
Registered Nurse currently licensed in the State of Florida.
BSN preferred
Hospice certification preferred.
Current Basic Life Support Certification.
Possess the ability to assess physical and psychosocial needs of patient and family.
Minimum of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
Understanding of and ability to apply characteristics of Hospice Nursing, case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
Basic computer skills and EMR software experience a plus.
Adequate and reliable means of transportation with adequate insurance and valid Florida driver\'s license.
