Family Nurse Practitioner Enhanced Primary Care
Salary ranges from $120,000.00 to $160,000.00 per year based on experience and education. Position Summary
The Article 28 Primary Care Clinic is expanding its services and seeking a qualified, compassionate Family Nurse Practitioner (FNP) to join our interdisciplinary healthcare team. The ideal candidate will be committed to improving access to high-quality, person-centered healthcare for individuals with intellectual and developmental disabilities (IDD). This role provides the opportunity to deliver holistic primary care within a clinic designed to meet the unique needs of this specialized population. Principal Duties and Responsibilities
Provide comprehensive primary and preventive care across the lifespan within an Article 28 framework. Conduct assessments, diagnose conditions, and develop evidence-based treatment plans. Collaborate with physicians, providers, and interdisciplinary teams to coordinate individualized care. Support health promotion, patient education, and chronic disease management tailored to individuals with IDD. Participate in quality improvement initiatives and contribute to the enhancement of clinic workflows and clinical protocols, with a focus on health equity and reducing barriers to care. Provide clinical leadership and mentorship to nursing and support staff, fostering a culture of compassion, compliance, and excellence. Maintain clinical documentation in accordance with regulatory standards and organizational policies. Deliver services both in-person and through telehealth as appropriate. Other duties as assigned. Qualifications, Skills and Knowledge Requirements:
Current New York State Family Nurse Practitioner license and certification (MSN or DNP required). Minimum of 3 years of clinical experience in primary care, family medicine, or internal medicine required. Experience with the IDD population is preferred but not required—training and support will be provided. Strong clinical assessment, communication, and interdisciplinary collaboration abilities. Commitment to providing inclusive, equitable, person-centered, and accessible care for individuals with IDD. Willingness to obtain required Intellectual and Developmental Disabilities (IDD)–specific certifications and complete related educational training. Duties require professional verbal and written communication skills. Meet the requirements to drive Springbrook vehicles as required.
