Responsibilities
Provides on-site brief intervention, de-escalation and crisis management and recovery support at designated LIHI-operated Tiny Home Village.
Becomes a visible, stable and accessible presence at assigned Tiny Home Village.
Through progressive engagement, works with Tiny Home Village residents to identify and begin to address mental health and substance use issues which may present barriers to long-term, stable housing.
Collaborates with on-site LIHI staff to respond to resident incidents and support resident compliance with Village rules and standards of behavior.
Offers individual and group counseling sessions while residents are in ideation (pre-enrollment) phase.
Performs Mental Health intake/evaluations and collects and processes psycho-social data from residents for purposes of clarifying treatment goals and establishing preliminary diagnoses that may be required to access housing or other services.
Provides a warm handoff to THS staff for residents who wish to pursue formal Mental Health or Substance Use Disorder Treatment.
Ensures prompt, accurate clinical documentation including treatment plans, progress notations in assigned participant files; preparation of treatment contracts, letters, referrals, discharge summaries of terminated participants as appropriate.
