The Physical Therapist (PT) - Home Health provides in-home physical therapy to help patients regain mobility, alleviate pain, and improve physical function. Working closely with patients and following a physician’s care plan, the PT leads exercises, muscle reeducation, gait training, and other functional activities to support recovery. Accurate documentation and collaboration with the healthcare team are essential to ensure continuity of care. This role is ideal for licensed physical therapists with a commitment to enhancing patient outcomes and offers flexibility and mileage reimbursement.
Responsibilities
Provide in-home physical therapy based on the physician’s care plan
Administer treatments to relieve pain, improve mobility, and enhance function
Lead therapeutic exercises, gait training, and other functional activities
Maintain accurate patient records and attend team meetings
Participate in professional development and educational programs
Qualifications
Licensed Physical Therapist in the state
Doctorate or Master’s degree in Physical Therapy
At least one year of experience as a Physical Therapist, preferably in home health
Current CPR certification
Valid driver’s license, auto insurance, and reliable vehicle for home visits
