Description Position Summary The Registered Nurse is a member of the interdisciplinary team working in collaboration with the Physician. The Registered Nurse utilizes the nursing care process consistent with their license and/or certification including initiation of culturally competent continuum of care including inpatient, outpatient and community settings. The Registered Nurse is responsible to oversee any necessary coordination of the interdisciplinary plan of care and provides for its consistency with the medical treatment plan. The patient care process includes:

Assessment – an orderly process consisting of the identification, gathering and organization of subjective and objective data pertaining to the patient/family/significant other, including physical, psychosocial, spiritual and cultural.

Planning – an organized step in the process that consists of the identification of actual or potential problems and desired outcomes (goals) that result in a documented plan of care. The plan of care provides direction for the patient/family/significant other and interdisciplinary team.

Intervention – the specific interventions or orders that are implemented consistent with the plan of care and medical treatment plan.

Evaluation – a step in the process that involves determining the effectiveness of the interventions in achieving the plan of care and determining the conclusions concerning the achievement or lack of achievement of desired outcomes. The evaluation may initiate the need for reassessment and revisions to the problems, goals, plan of care, and interventions. This step provides for documentation of required actions.

Based upon findings identified during assessment and reassessment:

Patients have problems identified and prioritized relative to biophysical, psychological, environmental, self care, educational and social status.

Goals are identified consistent with the problem to provide direction for patient care and health promotion.

Education is consistent with the identified problem and associated goals and is seen as critical to creating an environment that promotes patient empowerment.

Employee may be required to perform Limited Laboratory Services to include: patient identification and preparation for specimen collection, processing samples for testing, performing required quality control testing and instrument maintenance and function checks, monitoring, recording and reporting tests results to clinicians including critical values, and maintaining competency/certification of the aforementioned tasks and duties.

Licensed and registered care providers responsible for utilizing the care process relative to their scope of practice and knowledge base may document the assessment, plan, intervention and evaluation. All patient care activities are administered according to standards, procedural guidelines and organizational policy.

The Registered Nurse actively supports performance improvement initiatives and provides leadership to other members of the patient care team. By participating in educational activities, the Registered Nurse assumes responsibility for continued professional and personal growth.

Qualifications Education / License:

Graduate of an accredited school of nursing; BSN preferred. Current Licensure as a Registered Nurse in the State of New Jersey or current multi‑state license required. BLS required. Valid NJ Driver’s License is required for all employees in the Adult and Child Partial Programs.

Experience:

Proficiency in Clinical Applications preferred at the time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30–60 days from date of hire.

Specialty Experience, Advanced Certification Required / Certification Preferred Emergency Room City: 2–4 years medical/surgical experience preferred; 2 years emergency room experience preferred. ACLS required within 6 months of initial assignment and continuously maintained thereafter. All registered professional nurses after 24 months of initial assignment will have completion of ATCN or TNCC. Minimum of 8 contact hours of education every 1 year in basic trauma assessment, intervention and stabilization; PALS or ENPC with provider status attained within 12 months of initial assignment and continuously maintained thereafter. Minimum of 8 contact hours of education every 1 year in basic stroke assessment, intervention and NIHSS to be completed by 6 months of hire and continuously maintained thereafter.

Labor & Delivery / Obstetrics 2–4 years labor and delivery experience preferred. ACLS required within 6 mo of hire or transfer. NRP (NALS) required within 1 year from date of hire or transfer.

Neonatal Intensive Care / Intermediate Care 2–4 years NICU experience preferred. NRP (NALS) required within 1 year from date of hire or transfer.

Operating Room Minimum 1 year medical/surgical experience desired; 1–2 years operating room experience preferred. CNOR preferred. ACLS preferred.

Critical Care (ICU) 2–4 years medical/surgical experience preferred; 1–2 years critical care experience preferred. ACLS required within 1 year from date of hire or transfer. CCRN preferred.

Medical/Surgical 1–2 years medical/surgical experience preferred. Effective February 18th 2020, ACLS is required for all currently hired employees. Incumbents hired after December 31st 2021 will have 6 mos from date of hire or transfer to obtain ACLS.

Pediatrics 1–2 years medical/surgical experience preferred; 1–2 years pediatric experience strongly preferred. PALS required within 1 year from date of hire or transfer.

Radiology / Cath Lab 2–4 years medical/surgical experience preferred; 1–2 interventional experience preferred. ACLS, PALS required within 1 year from date of hire or transfer.

Oncology / Chemotherapy 1–2 years medical/surgical experience preferred; chemotherapy experience preferred. All employees within job codes are required to ACLS; within 1 year from date of hire or transfer. Chemotherapy certification preferred.

Performance Expectations Demonstrates the ability to perform the department specific competencies as listed on the Assessment and Evaluation Tool.

Work Environment Potential for exposure to the hazards and risk of the hospital environment, blood borne pathogens, including exposure to infectious disease, hazardous substances, and potential injury. Includes considerable reaching, stooping, kneeling, crouching, extensive standing and walking, lifting 25–100 pounds, pushing and pulling heavy equipment, and in some departments the ability to distinguish colors.

Unit Populations Served Core competencies will be assessed primarily on the following patient populations: Bariatric, Cath Lab, Critical Care, Behavioral Health, Dialysis, Endoscopy, Emergency Department, Intermediate Care, Medical Surgical, Telemetry, Wound Care, Structural Heart, and others as defined by each unit.

Reporting Relationship This position reports to the patient care unit or department leadership.
