Intake Coordinator Hospice – Affinity Health Management Responsible for managing all aspects of the patient intake process, including leading the intake team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing insurance verification and authorization processes.

Essential Job Functions / Responsibilities

Direct all daily patient referral and intake operations with direct oversight of intake policies.

Enter referral/patient information into electronic medical record system and other applications as needed.

Coordinate with referral sources, marketing, and clinical teams to ensure patient transition into hospice care.

Ensure compliance with all state, federal, and CHAP referral/intake regulatory requirements.

Implement improved work methods and procedures to ensure patients are admitted in accordance with policy.

Establish and maintain positive working relationships with current and potential referral sources.

Oversee insurance verification and authorization processes.

Maintain comprehensive knowledge of Continuum Care’s contractual relationships and ensure patient admission according to contract provisions.

Maintain knowledge of community resources and assist referral sources in accessing them if services are not available from Continuum Care.

Position Qualifications

High School Diploma required; College Degree preferred.

Proficient in Google Drive, Sheets, Docs preferred.

Minimum of three (3) years’ experience in health care, preferably in home care operations.

Demonstrated ability to supervise and direct professional and administrative personnel.

Ability to market aggressively and deal tactfully with customers and the community.

Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices.

Demonstrates good communication, negotiation, and public relations skills.

Displays autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
