Job Description Summary

Responsibilities
Conduct comprehensive patient assessments, including vital signs, medication administration, wound care, and disease management.
Develop individualized care plans that address patients’ specific needs and goals.
Educate patients and their families on disease management, medication administration, and self‑care techniques.
Provide direct patient care, including wound care, medication administration, injections, and IV therapy.
Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
Maintain accurate and timely documentation of patient assessments, interventions, and progress notes.
Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.
Requirements
Current and valid RN license in the state of Florida.
Minimum of 1 year of experience in a hospital or skilled nursing facility.
Strong clinical skills, excellent communication and interpersonal abilities, and a patient‑centered approach to care.
Strong work ethic, reliability, and a commitment to providing high‑quality care.
Reliable transportation to visit patients in their homes.
