Job Description
$3,000 Relocation available for eligible candidates (external only, 12‑month contract required, relocation must be greater than 50 miles for employment at CFD facility).
Location: 600 Courtland Suite 110, Maitland, FL 32804
Travel up to 75%
Ensures timely communication with patients, their families, and healthcare providers through professional collaboration.
Collaborates with the Community Care team to evaluate patients' medical and psychosocial conditions and assess their needs.
Qualifications Master’s (Required). Clinical Social Worker License (LCSW) – EV Accredited Issuing Body. Driver's License (DL) – EV Accredited Issuing Body.
Pay Range $54,230.45 – $100,875.88
