Position Information The Community Health Worker (CHW) Supervisor will be responsible for overseeing the Center's per diem Community Health Workers and provide support to the identification and coordination of community member's health and social service needs. The Community Health Worker Supervisor will be responsible for determining appropriate outreach and scheduled activities for the Center that require the assistance of CHWs. In addition, the CHW Supervisor will be responsible for supporting community members in improving their health through outreach and engagement activities either via phone and/or in the field and connecting them with appropriate social service organizations.
Please Note: This position is a career grant-funded appointment with an end date of 06/30/2024 and may be extended should funding be available.
Applicants must have current work authorization when accepting a UCR staff position. Currently, we are unable to sponsor or take over sponsorship of an employment Visa for staff.
As a university employee, you will be required to comply with all applicable University policies and/or collective bargaining agreements, as may be amended from time to time. Federal, state, or local government directives may impose additional requirements.
Education
Degree:
Associate degree in related area and/or equivalent experience/training.
Requirement:
Continuing education in related field. Completion of an approved Community Health Worker program.
Preferred.
Certifications
Requirement:
CHW State Certification when available.
Preferred.
Experience Requirement:
4 - 7 years of related experience.
Minimum Requirements
Valid Drivers License
Travel Required
Preferred Qualifications
Skill in using business software systems in the completion of work assignments; i.e. Word, Excel, Access, PowerPoint.
Thorough skills to conduct needs assessment and development of new presentations/courses to meet organizational needs.
Thorough knowledge of and skill in delivering programs.
Thorough knowledge of functional area and understanding of how work may impact other areas.
Thorough knowledge of current community educational programs and trends.
Knowledge of managing a portfolio of business and management courses at the university level.
Thorough analytical skills to conduct analysis and develop recommendations, demonstrating organization and problem-solving skills.
Thorough knowledge of organization policies and procedures.
Thorough and effective verbal, written and interpersonal communication skills and presentation skills.
Additional Information
Effective communication skills to interact with various members of the community.
Ability to assess external and internal environments.
Bilingual skills in English and Spanish.
