Job Description Metro is looking for a motivated and passionate
PER DIEM

Job Type: Per Diem
Pay: $65–$80 per visit
Location: In and around Plainview
Schedule: Flexible

Responsibilities
Accept and treat patients within your designated geographic area
Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
Develop a plan of care and treatment goals in consultation with the physician, patient, and family
Plan, prepare and implement customized therapy programs to restore function and promote independence
Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
Provide timely and accurate documentation for each visit
Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
Deliver care in compliance with agency policies and professional practice standards
Qualifications
Physical Therapist Program Graduate
Licensed Physical Therapist in the state of New York or eligible
Mandatory certifications: First Aid, AED, and CPR
Home‑health experience preferred but not required – a mentor will be provided to help with documentation and compliance
Ability to travel to patients’ homes within the assigned geographic territory
