Base pay range $37,440.00 per year – $43,800.00 per year
Responsibilities
Provides direct patient care under RN guidance according to the Aide Plan of Care (POC).
Assists with patient medications, including opening caps, reading labels, verifying doses, and observing administration.
Accurately measures temperature, pulse, blood pressure and reports abnormal results to supervisor.
Maintains patient personal hygiene, performing or supervising bathing, grooming, skin care, shaving, oral care, nail/foot care, etc.
Assists in feeding patients, communicates nutrition principles, observes and records intake, positions for meals, assists in self‑feeding.
Assists with toileting, use of bedpan/urinal, changes and positions of catheter bags and procedures for regulated ostomies.
Safely assists with patient mobility, exercises, positioning/turning, transfers and ambulation per Plan of Care and policy.
Reports changes, documents interactions with patients, caregivers, doctors, and staff accurately and legibly.
Practices infection control and maintains clean, safe environment.
Assists with light household duties (laundry, bed changing, meal prep, housekeeping, shopping) when no alternative assistance is available and under MD order.
Qualifications
High school diploma or equivalent.
Completion of Certified Nursing Assistant (CNA) or Certified Home Health Aide (HHA) program within last 24 months.
State certification required.
Valid driver’s license, auto insurance, and reliable transportation.
In good standing on HHA Registry (if applicable) and completed HHA/CNA course for Medicare‑certified agency.
At least one year experience in last 24 months as a Home Health Aide or CNA in hospital, nursing home, or home‑health/hospice agency.
Schedule 40 hours per week (full‑time)
