Job Summary As an

Admissions Coordinator

at Accolade Healthcare, you will manage the admissions process for residents in our skilled nursing facility. Your role involves coordinating with prospective residents, their families, and clinical and administrative staff to ensure a seamless transition into the facility. This position requires strong organizational skills, compassionate communication, and a proactive approach to supporting residents and their families.

Duties and Responsibilities

Admissions Process Management:

Collaborating with the Accolade Healthcare Intake Department to process referrals in a timely manner.

Screening prospective residents at local healthcare facilities and serving as a point of contact for clinical and financial inquiries.

Conducting facility tours for prospective residents and their families, addressing questions, and providing detailed information about services.

Assessment and Documentation:

Working with healthcare professionals to assess prospective residents' needs.

Preparing, explaining, and executing admission documents for residents and their families.

Maintaining medical records and ensuring compliance with documentation standards.

Communication:

Acting as the primary liaison for families during the admissions process, providing updates and support.

Coordinating with referral sources, including hospitals and rehab centers, to facilitate timely admissions.

Data Management:

Maintaining logs and records for accurate monthly reporting.

Analyzing admission data to identify trends and improve processes and resident experiences.

Customer Service:

Providing exceptional service to prospective residents and families, addressing concerns promptly.

Fostering a welcoming environment that respects resident dignity and comfort.

Other duties as assigned.

Qualifications and Skills

An associate degree in healthcare administration, social work, or a related field (preferred).

Previous experience in admissions, customer service, or healthcare settings, especially in skilled nursing or long-term care (highly desirable).

Strong communication and interpersonal skills with the ability to empathize with residents and families.

Excellent organizational and multitasking abilities with attention to detail.

Proficiency in PointClickCare, Microsoft Suite Applications, and Canva (highly desirable).

Knowledge of healthcare regulations and compliance related to skilled nursing facilities (highly desirable).

Work Conditions

Flexibility to work occasional evenings and weekends to accommodate family schedules or special events.
