Millennium Home Care (an affiliate of Millennium Healthcare, LLC) is seeking dedicated PRN Speech–Language Pathologist (SLP) to join a leading in‑home healthcare provider. Conduct comprehensive evaluations of patients’ speech, language, cognitive‑communication, and swallowing abilities to identify strengths, weaknesses, and treatment needs. Develop individualized treatment plans addressing specific goals in speech sound production, language development, fluency, voice disorders, and swallowing difficulties. Deliver direct therapy services such as articulation, language, fluency, voice, and swallowing therapy using evidence‑based practices. Educate patients and families on strategies to improve communication skills, swallowing safety, and overall quality of life. Collaborate with physicians, occupational therapists, and physical therapists to create comprehensive care plans and ensure optimal patient outcomes. Maintain accurate and timely documentation of evaluations, treatment plans, progress notes, and discharge summaries. Adhere to all regulatory standards and guidelines, including HIPAA and Medicare regulations. Requirements
Current and valid Speech‑Language Pathologist (SLP) license in the state of Florida. Minimum of 1 year of experience in a hospital, rehabilitation center, or skilled nursing facility. Strong clinical skills, excellent communication and interpersonal abilities, and a patient‑centered approach to care. Strong work ethic, reliability, and commitment to providing high‑quality care. Reliable transportation to visit patients in their homes. Benefits
