Registered Nurse (RN) PRN – Experienced Home Health with Oasis Documentation – Visits – Hillsborough County
TGH Home Care powered by VNA of Florida provides a full array of home care services for TGH patients and those in the community needing home care. This office continues the same level of care and experience received at Tampa General Hospital. We are seeking an exceptional Registered Nurse to join our team.
Responsibilities
Develop work methods and procedures that facilitate the rendering of high-quality care.
Deliver professional nursing services in the client’s home setting and provide case management for their primary patients.
Monitor the changing needs of the client and or family.
Provide initial assessment, oversee progress toward goals, and monitor clinical outcomes.
Maintain ongoing communication with patient, their families, physicians, other disciplines, the Clinical Manager, and office staff.
Participate in staff interdisciplinary conferences.
In conjunction with the patient and/or their representative develop, implement, and continuously monitor the plan of care to ensure quality of care for patient.
Complete clinical documentation, including OASIS, evaluations, orders, and documentation of care coordination.
Work with physicians and discharge planners while promoting health and education to patient and/or caregiver.
Function as a liaison with the community’s health‑related resources including DME, oxygen, and infusions.
Interact with patients and families, team members, and other health care professionals.
Participate in ongoing education, seeking opportunities for continuous personal and professional development.
Requirements
Active Registered Nurse license required.
Minimum of 1 year clinical experience.
Home health/HCHB experience.
