Role Description This full-time Case Manager role for a Registered Nurse is based in Knoxville, TN. Our Knoxville office covers the Knox County area only! As a Registered Nurse, you will provide compassionate, patient-centered hospice care to individuals with life-limiting illnesses. Responsibilities include assessing patient needs, creating personalized care plans, administering medications, monitoring patient progress, and maintaining communication with families and the care team. You will also ensure compliance with healthcare standards and provide emotional support to patients and their loved ones.
Qualifications
Current registered nurse (RN) license in the state of Tennessee and good standing with the state board of nursing.
Skills in patient assessment, the development and implementation of care plans, medication administration, and symptom management.
Ability to communicate effectively with patients, families, and interdisciplinary teams, demonstrating compassion and empathy.
Organizational skills to manage multiple patient caseloads and maintain accurate documentation.
Knowledge of end-of-life care practices and hospice regulations is preferred.
Proficiency in operating electronic health record (EHR) systems and basic computer skills.
Reliable transportation and a valid driver’s license to travel locally as needed for patient care.
A minimum of 1-2 years of nursing experience in hospice, palliative care, or relevant medical settings is a plus.
