Kolmac Integrated Behavioral Health & Concerted Care Group (CCG) is looking for a dedicated Mental Health Therapist to join our team in Dover, DE. This role offers a supportive, patient-centered environment where your expertise is truly valued. What You’ll Do
On-Site Clinical Presence Monday - Friday 7:00 AM - 3:00 PM at our Dover, DE location Provide individual and group therapy, assessing and tracking clients’ stages of change readiness and recovery Complete assessments, progress notes, treatment plans, and billing documentation within 24 hours in compliance with Medicaid, Medicare, and CARF standards Build mutual, collaborative, and individualized treatment alliances, adapting to clients' changing clinical needs Regularly assess treatment progress with clients and the clinical team Connect patients to support services, referral sources, family members, and other providers to ensure holistic care Engage in assessments, collaborative treatment plans, BAMs, discharge planning, and partnerships with referral sources & providers Participate in staff meetings, clinical team meetings, therapy supervision, and in-service training Maintain confidentiality and ensure all client records, treatment plans, and communications meet professional, ethical, and legal standards Work independently and efficiently, meeting deadlines and maintaining a high level of professionalism Stay current with best practices, research, and treatment strategies in mental health care What You Bring to the Table
Master’s degree in social work, counseling psychology, or a clinically related field Active Delaware license in good standing (LCSW or LPCMH) Medicaid Payee # and CAQH account (required for employment) Experience with adult clients in a behavioral health or substance use setting Proficiency in electronic health records (EHR) and clinical documentation A passion for helping adults on their journey to recovery and well-being What We Offer
