Nurse Practitioner: Collaborative with Medical Director SNF

Responsibilities: Act as the Skilled Nursing Facility’s leading provider of internal medicine services. Collaborate with the interdisciplinary team and physicians, serving as a day-to-day clinical leader. Oversee care of all residents, both short- and long-term, working with nursing staff and medical directors. Provide documentation, evaluation of new-admission visits, coordination of discharge visits, monthly stability visits, evaluations for changes in condition, and advanced care planning. Conduct comprehensive health assessments, differential diagnosis, prescribe treatments, perform physical exams, develop and implement patient management policies, facilitate referrals, and communicate with attending physicians. Requirements
Advanced Practice Licensure (APN) with applicable state license. Active license and DEA number. Previous experience in nursing or other medical fields. Minimum one-year experience in a Skilled Nursing setting (preferred). Ability to build rapport with patients and loved ones. Ability to thrive in a fast‑paced environment. Excellent written and verbal communication skills. Strong leadership experience emphasizing teamwork. Education
Master’s degree required. Salary: $105,000 – $120,000 (subject to experience).
