Trinity Employment Staffing Agency is looking for a Nurse Practitioner for a direct hire opportunity! Job Summary Our client is an innovative, rapidly growing medical group that is changing the way primary care is delivered to seniors. Our state-of-the-art centers are in communities where there is little to no quality healthcare today. The healthcare model is entirely driven by providing better primary care to our patients, not by the volume of services we provide. Because we are fully accountable for the health of our patients, we spend more time with them and offer more comprehensive care. Put simply, we provide the care that our patients deserve.
Advantages for Nurse Practitioners & Physician Assistants
Employed position with above-market compensation that includes base salary and bonus (based upon quality metrics, not RVUs or volume)
Outpatient primary care responsibilities only
Full malpractice coverage with tail

Ideal Candidates
Currently licensed to practice professional nursing or Physician Assistant in the state of employment, active and non-probationary
Experienced in primary care: internal medicine, geriatrics, or family medicine
Current BLS / ACLS certification
Driven to and comfortable with managing a primarily Medicare-aged, medically underserved patient population with comorbid conditions
Strong understanding of examination methodologies and diagnostics
Able to demonstrate interpersonal skills to be approachable and understandable to patients, families, and center team members
Confident in their ability to deliver high quality primary care, which includes being available to patients and addressing their medical and social needs
Passionate about their work and energized by being part of a fast growing organization
Comfortable traveling within the market to multiple centers within short distance of each other

Qualifications & Experience Currently licensed to practice professional nursing or Physician Assistant in the state of employment, active and non-probationary
Experienced in primary care: internal medicine, geriatrics, or family medicine, minimum of 1 year active experience
Responsibilities
Diagnose and treat acute, episodic, or chronic illness, independently or as part of a healthcare team. May focus on health promotion and disease prevention. May order, perform, or interpret diagnostic tests such as lab work and x rays. May prescribe medication. Must be registered nurses who have specialized graduate education.
Maintain complete and detailed records of patients' health care plans and prognoses.
Develop treatment plans, based on scientific rationale, standards of care, and professional practice guidelines.
Provide patients with information needed to promote health, reduce risk factors, or prevent disease or disability.
Analyze and interpret patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
Diagnose or treat complex, unstable, comorbid, episodic, or emergency conditions in collaboration with other health care providers as necessary.
