Essential Functions

Delivery of Patient Care

Assumes responsibility for the coordination and delivery of patient/caregiver care.

Conducts nursing assessments to assist in the overall management of patient/caregiver needs.

When appropriate, develops, implements, evaluates, and updates the plan of care to ensure that care is uninterrupted and that goals are achieved.

Facilitates patient and caregiver independence to the extent possible.

When providing hospice care, appropriately utilizes VIA Health Partners patient‑related protocols, standing orders and guidelines.

Assumes responsibility for conducting effective patient and caregiver education.

Encourages and allows time for questions and consultation.

Provides education to patients and families as needed.

Communicates effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care.

Maintains open lines of communication with supervisor. Keeps supervisor apprised of any complex patient/caregiver issues.

Applies principles of infection control. Follows guidelines for universal precautions in accordance with accepted professional practices.

Interdisciplinary Group

Attends, participates, and may facilitate interdisciplinary group conferences [IDG] and special care conferences.

Assumes responsibility for establishing and maintaining effective working relationships with team members.

Identifies specific patient/caregiver needs and makes referrals to appropriate team members.

Documentation

Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.

Professional and Personal Accountability

Demonstrates competency in knowledge and skills.

Ensures that VIA Health Partners professional reputation is maintained and projected.

Assumes responsibility for professional development and staying abreast of current trends in the healthcare field. Incorporates new information and methods into practice.

Maintains current professional licensure, CPR certification and CEU requirements.

Maintains productive and professional relations with external agencies and healthcare providers.

Exhibits time management skills. Able to plan workday while remaining able to respond appropriately to crisis.

May perform other duties as required

Cleans patient rooms as required per patient room cleaning procedure. Ensures all areas of facility are clean and appropriately stocked with supplies. Keeps the family room clean throughout the day. Ensures proper chemicals are used for cleaning different surfaces. Ensures protective equipment is used when working with chemicals, i.e., gloves, glasses.

Qualifications

A diploma or associate degree in nursing is required; bachelor’s degree preferred.

A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.

The Nurse Licensure Compact (NLC) allows an RN and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency and to practice in other compact states while subject to each state’s practice laws and discipline.

Current CPR required; CHPN preferred.

Minimum 1‑year related experience for inpatient units (IPU) and 2 years for other nursing positions. Experience in direct care of patients with life‑limiting illnesses required. Related experience may include hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, medical surgical.

Computer proficiency is required.
