Education and Experience

Wisconsin Licensed Registered Nurse; Bachelor of Science preferred, in compliance with the Federal Nurse Practice Act.

Minimum 50% of working hours standing or moving; ability to sit, stand, bend, lift, and move intermittently.

Functional sight, hearing, speech, tactile sense required to complete essential job functions; ability to read, write, speak, and understand English.

At least 3 years of nursing experience, with recent experience in geriatric nursing.

At least 1 year of MDS experience.

Ability to cope with mental and emotional stress of the position, good general health, and emotional stability.

Ability to lift, push, pull, and move a minimum of 50 pounds.

Ability to assist in the evacuation of residents.

Purpose of the Job Position Plans, coordinates, supervises, and implements nursing services for residents as part of the geriatric interdisciplinary team. Completes the MDS and Medicare Documentation and Assessment Forms based on state and federal regulations for long‑term care.

Responsibilities

Makes clinical decisions to guide effective geriatric nursing practice, utilizing physician direction and nursing judgment; assists in direct resident care such as handling or moving residents.

Assesses residents’ health status and communicates information with appropriate members of the health care team; uses nursing process and procedures to meet resident needs.

Coordinates and participates in planning and implementation of admissions and discharges, ensuring level of care status upon admission and quarterly updates.

Coordinates all disciplines in the resident assessment and care planning process.

Ensures a comprehensive plan of care is developed and reviewed as necessary; incorporates new orders and changes in condition; communicates changes to team members.

Coordinates documentation of the MDS and Medicare Assessments by the interdisciplinary team.

Heads or delegates care conferences; maintains liaison with residents, families, other departments, and agencies.

Documents resident care and assessments using common medical terms and approved abbreviations.

Supervises and directs team members, utilizing facility policies and procedures; demonstrates leadership and supervisory skills.

Prepares and administers medication and treatments per medical orders.

Maintains resident confidentiality and privacy.

Responds to emergencies and life‑threatening situations; provides assistance as required.

Engages with residents, personnel, visitors, and government agencies under all circumstances.

Participates in staff development programs and orientations of new staff.

Keeps abreast of latest developments by attending workshops and reading professional journals.

Works beyond normal hours, including weekends and holidays, when necessary.

Compensation Salary: $38.00 – $41.00 per hour
