$10,000 Salary:

$120,000 – $140,000 per year Full benefits package.

Your Role

Provide in‑home, patient‑facing primary care visits, initial screens, physical exams, and timely documentation.

Partner with clinicians in the Care Community to manage the health and wellbeing of patients in their homes.

Engage attributed patients in the WellBe program and support ongoing care needs to keep them engaged.

Facilitate decision‑making for staff, patients, and families by offering educational resources.

Address urgent care needs, including transition of care visits.

Collaborate with the community team to complete full patient care assessments and plans.

Order, interpret, and analyze diagnostic tests; determine follow‑up per state supervisory agreement.

Prescribe and manage medications, including psychopharmacological agents, per state supervisory agreement.

Update patient records and verify accuracy at each appointment.

Deliver patient care and conduct regular reviews with supervising physician per state supervisory agreement.

Focus on documentation/coding, HEDIS measures, patient engagement, and medical expense management.

Utilize available resources and refer to internal/external resources, such as WellBe behavioral health protocols.

Collaborate with interdisciplinary team and attend regularly scheduled team meetings.

Perform additional tasks needed to accomplish team objectives and goals.

Qualifications

Master’s degree in Nursing (Family, Adult, Geriatric, or Adult‑Geriatric Nurse Practitioner) from an accredited program.

Active, unencumbered Nurse Practitioner license in the state of posting and residency requirement.

Current board certification (ANCC or AANP).

BLS certified and active NPI# as an NP.

CPR certification.

Active, unencumbered DEA licensure or eligibility to obtain.

Valid driver’s license, car insurance, and willingness to drive to patient homes.

Proof of active auto insurance policy.

Skills & Experience We Value

3+ years Nurse Practitioner experience (preferred); new graduates considered.

Prior healthcare roles such as medical assistant, CNA, EMT/paramedic, lab assistant, RN, or related experience.

Experience in value‑based care, home care, primary care, hospice, palliative care, or geriatrics (preferred).

Experience delivering care via telehealth (preferred).

Ability to conduct in‑home visits and use telehealth platforms.

Proficiency with Microsoft Office, Electronic Health Records, scheduling software, and Ring Central.

Additional Requirements

Position is safety‑sensitive; full cognitive and physical functioning required.

Subject to drug and alcohol testing in accordance with federal and state laws.

Pre‑employment drug screening required; ongoing employment contingent upon compliance.

Travel & Work Conditions

Travel may be required up to 100% locally to deliver patient care.

Ability to lift up to 30 lbs.; moving/lifting patients may involve up to 50 lbs. or more.

Ability to drive to patient locations (home, hospital, SNF, etc.).
